Characteristics of Recurrent Kawasaki Disease

Abstract
Kawasaki disease (KD) is one of the most common vasculitis of childhood and may cause acquired heart disease. The known recurrence rate is 1% to 3%. Despite the high rate, the risk factors associated with recurrence remain unclear. This study was performed to investigate the clinical characteristics of recurrent KD. We retrospectively reviewed the medical records of KD cases from January 2002 through July 2014 at the Chung-Ang University Hospital in Korea. A total of 358 KD children were enrolled. Of these, 15 (4.19%) were included in the recurrent KD group and 80% of recurrences were within the first 2 years. Laboratory data were nonspecific; hemoglobin was significantly increased in the second event of recurrent KD (p <0.05). Responsiveness to intravenous immunoglobulin (IVIG) was similar in the 2 groups; however, the number of coronary artery lesions (CAL) was significantly higher in the recurrent KD group (p <0.05). The recurrence rate seemed to be higher than in previous studies. There were no clinical symptoms or laboratory findings predictive of the possibility of recurrence. The responsiveness to IVIG was similar in all groups; however, higher the number of relapses, higher is the chance of coronary artery complications.
